This position involves conducting psychosocial assessments, developing individualized treatment plans, and delivering evidence-based therapeutic interventions to patients across the lifespan. As part of an integrated care team, the LISW will collaborate closely with primary care providers, nurses, and other behavioral health professionals to ensure coordinated, whole-person care. The ideal candidate will have a strong commitment to serving underserved populations and possess the clinical skills necessary to address a wide range of behavioral health issues, including depression, anxiety, trauma, substance use, and adjustment disorders. Experience working in rural or medically underserved settings is a plus. Strong interpersonal, organizational, and documentation skills are required.
